Year Program Started: As many of the AIDS Service Organizations in San Francisco, the Mission opened in the early 1980's, in 1984. The first RD was hired in 1997.

Location & Telephone Number: Shotwell Street, San Francisco, CA  94110; 415/431-3212, ext. 240.

Number of Days/Hours RD is on Site: 15 hours (3 evenings of 5 hours per week)

Funding Source:  Clinica Esperanza gets its funds mainly from three agencies. The State of California Office of AIDS Early Intervention Program (EIP), Ryan White CARE Act Title I and Title III funds, and the San Francisco Department of Public Health General Fund.

Is someone involved in program operation also actively involved with your local HIV/AIDS Health Services Planning Council?  Yes, the Health Educator at the Clinic.

Patient Referral Source: The Mission's Medical Department Director, Case Managers and Clinic Nurse, word of mouth from various sources, the county hospital, and San Francisco city clinics refer people to the facility.

Number of Clients and Specific Population Served: Mission Neighborhood Health Center- Clinica Esperanza serves 435 adults; 86% of them are Latinos: 90% men, 6% women and 4% transgender.

When are patients seen? The Medical Clinic hours are Monday-Thursday 5:30- 9:30 p.m. and Friday from 9-6 p.m. Case Managers are available from 9 a.m. to 9 p.m.

Are all patients screened for nutritional risk? Yes, upon initial clinic assessment.

How often do you provide nutritional assessments and follow-ups? These are provided upon initial team assessment and then quarterly, or as needed or requested.
